Instructions: To provide important background information plus to reduce time completing paperwork at the time of the first session, please print out the appropriate forms, complete, and bring with you to the first appointment.

 

Child or Adolescent Initial Evaluation (2 forms)
        • Information Sheet   (name, address, etc.)
        • Developmental History (Child & Adolescent)

 

Adult Initial Evaluation (2 forms)
        • Information Sheet   Adult form (name, address, etc.)
        • Adult History Form   (Note for Couples/Marital: Print two copies of the Adult
                                          History form—one for each partner to complete)
